  • We re-opened our new patient waitlist on June 10th, 2024. If you are interested in joining our CVFP Health Home, please fill in and submit our online form. You can find it at cvfp.com, New Patients page or by clicking on the link below.

  • CVFP is located in Calgary at the Crowfoot West Business Centre, 600 Crowfoot Crescent NW.

    Other health services providers located in the same building as our clinic include a pharmacy, diagnostic imaging and optometry.

  • The CVFP clinic is open Monday to Friday from 8:00am to 5:00pm. We are closed Saturdays, Sundays and all statutory holidays.

  • Parking is conveniently located at the front entrance of our building at Suite 210, 600 Crowfoot Crescent NW, Calgary, Alberta.

    We offer accessible, free daily 3-hour parking. All you need to do when you arrive for your appointment is register your license plate number on tablets located in our building lobby or main clinic reception area. Additional time on top of the free 3 hours can also be purchased.

    Please note that vehicles not registered may be ticketed or towed.

  • It’s always our priority to keep staff and patients safe, while also ensuring we align our precautions with the latest evidence and guidance. If you have an in-person appointment at our clinic, please note the following:

    1) If you develop any illness symptoms before your scheduled appointment, please contact us.

    2) Our reception team will complete an illness screening at check-in.

  • CVFP does not currently offer walk-in appointments.

  • We know that life happens and sometimes appointments need to be cancelled or rescheduled. Ideally, we would appreciate a 24-hour notice. We understand that this is not always possible, however, it does allow us to free up your visit time for another patient.

  • CVFP’s team-based care approach means our patients have access to integrated health teams who work alongside CVFP doctors.

    Our health teams include a Physician Assistant, Registered Nurses and many other health professionals including Pharmacists, a Behavioural Health Consultant, Respiratory Educators and a Health Management Nurse. In addition, CVFP offers access to a Nurse Practitioner who specializes in women’s health.

  • Your care is delivered by an integrated Health Team. While we always try to book you with your own doctor or NP, if they are not available and you have an urgent care need, any one of the doctors from within your Health Team may see you.

    When CVFP is closed, you can access an After-Hours Clinic (located in NW Calgary) by calling 811. A nurse will discuss your health concerns and help you to book an appointment. You will not be required to wait as you would for a Walk-in Clinic. Your CVFP doctor will receive a report after your visit and will facilitate any follow up.

  • CVFP is a teaching clinic. You may be seen by a medical student or a family medicine resident.

    CVFP Residents are doctors working towards a specialization in family medicine. They work with patients under the supervision of your family physician for two years, with increasing independence. Many of our physicians completed their residency at CVFP.

    Medical Students are training to be doctors and some may become family physicians or specialists. They cannot learn to be doctors without you. Students work with CVFP patients under the direct supervision of our physicians.

  • Your health information is collected and protected under Alberta’s Health Information Act (HIA). HIA strikes a balance between the protection of privacy and enabling the appropriate amount of information sharing to provide health services.

    Questions relating to the access and privacy of your health information should be directed to the CVFP Privacy Officer at 403-208-0722, ext 549.

  • Health information for patients 16 years of age and older can only be shared with signed consent. This includes all family members -- parents, spouses and children. Signed consents can be sent using Brightsquid secure email or dropped off in-person at our clinic’s main reception.

  • CVFP is publicly funded.

    CVFP has been funded under a unique model for over two decades -- Alberta Health pays us differently than most physicians. Typically, when a Family Physician provides care for a patient, they bill Alberta Health a fee for that visit, and for each time they see that patient. This is called fee for service.

    At CVFP when a patient chooses to join our practice, Alberta Health pays us an annual fee to provide care for that patient, no matter how often we see them for a visit.

    There are a variety of physician funding models throughout Calgary and in Alberta. Our funding is unique to CVFP and is the foundation of our Health Home.

  • CVFP is publicly funded by Alberta Health and you do not pay for the cost of routine medical care.

    CVFP does charge for services which are not covered by provincial health insurance. Fees for these uninsured services follow guidelines from the Alberta Medical Association, and include sick notes and insurance requests. CVFP offers one- and two-year subscription options to cover some of these services.

  • CVFP's care delivery is the foundation of our Health Home. Care delivery is based on integrated Health Teams working together to know patients best and to provide thorough and comprehensive patient-partnered care. CVFP’s unique funding model supports our high standard of care.

    In an independent case study by the Health Quality Council of Alberta (HQCA) 2019, the CVFP model was found to be cost-effective when considering health system costs including emergency visits and in-patient hospital stays.
